Case report of orthotic and prosthetic care of a patient with a diagnosis of transtibial amputation of the tibia
Rybová, Pavla ; Neuwirthová, Svatava (advisor) ; Šolc, Zdeněk (referee)
vi Abstract Title: A case report of orthotic and prosthetic care of a patient with a diagnosis of transtibial amputation of the tibia Aims: The aim of this bachelor thesis is to discuss in theoretical terms the individual aspects of the case report of a patient that led to transtibial amputation of the right leg. In the practical part, a description of the production of the primary equipment is presented. Methods: In the theoretical part, the professional framework of the problem is presented based on a literature search, knowledge from professional seminars and my practical experience. The practical part describes the course of the production of the first equipment, from the collection of the measuring materials, through the production of the stump bed, tests and adjustments, to the handover of the aid. This is followed by the experiment of fitting an individual with a calceotic aid to complement the compensatory aid. The practical part is documented verbally and photographically as it took place during my professional practice. Results: Too early production of a provisional bed cannot reflect the rapid volumetric changes of the stump and leads to the need to produce a new bed in a very short time frame. Keywords: transtibial amputation, cushionings, tibiofemoral angle, diabetic foot syndrome
Case Study of Physiotherapy Treatment of a Patient with the Unilateral Transmetatarsal Amputation
Pěničková, Anežka ; Kučerová, Ilona (advisor) ; Valentová, Zuzana (referee)
Author: Anežka Pěničková Supervisor: Ilona Kučerová, M. A. Title: Case Study of Physiotherapy Treatment of a Patient with the Unilateral Transmetatarsal Amputation Objectives: The objective of this bachelor's thesis is an overview of information about diabetes in connection to lower limb amputation. It presents an in-depth case study of a patient with this diagnosis and her physiotherapy. Methods: This bachelor's thesis consists of two parts. The first part is a theoretical introduction with an overview of the body of knowledge about diabetes mellitus, its treatment, and about the diabetic foot syndrome. Furthermore, this part is a comprehensive introduction into the problem of lower limb amputation and the care patients with this diagnosis receive. The care is viewed through the prism of physical therapy, which can be beneficial also when complications associated with amputation occur. The second part offers a case study focused on physiotherapeutic care of a patient with the unilateral transmetatarsal amputation diagnosis. It includes the initial kinesiological analysis, detailed description of the therapy units, the kinesiological analysis at the end of the therapy and the evaluation of the outcome. The case study resulted from the author's practical training at the local hospital in Kladno...

Orthotic and prosthetic equipment for patients with diabetes
Humpálová, Ludmila ; Půlpán, Rudolf (advisor) ; Čichoň, Rostislav (referee)
Author: Ludmila Humpálová Title: Orthotic and prosthetic equipment for patients with diabetes Objectives: The main objective of the thesis is to synthesise the information from the available literature and to deliver a summary of the basic of orthotic and prosthetic equipment used for patients with diabetes. Another objective is a description of equipment's basic manufacturing processes , mainly used for patients with diabetes. This thesis should help to clarify the most common options for equipment of patients with diabetes. Methods: The basis of the analytical part is literature review. In the practical part I have consulted my findings with experienced prosthetic. During the continuous practice I process an inventory of equipment options to patients with diabetes. Results: The result is a summary of information retrieval in equipment of patients with diabetes. Keywords: prosthesis, orthesis, syndrome of diabetic feet, arch supporter
Mikrocirculation in the diabetic foot syndrome
Tomešová, Jitka ; Lacigová, Silvie (advisor) ; Chlup, Rudolf (referee) ; Prázný, Martin (referee) ; Jirkovská, Alexandra (referee)
Abtrakt (EN) In spite of the fact that the diabetic foot syndrome is not considered a marginal complication of the patients with diabetes mellitus any more, its prevalence and incidence is not decreasing. The fact that early diagnosis and treatment or even prevention of foot ulcers is associated with a decrease in healthcare costs and, more importantly, with reducing patients' suffering, applies also in this case. The dissertation work originated in the Diabetologic Center of the Medical department I., University Hospital and Medical Faculty in Pilsen, Charles University in Prague, which has been dealing with diagnostic and therapeutic procedures in patients with diabetic foot syndrome for over 20 years. The examinations for diagnosis of the main etiologic factors, i.e. peripheral neuropathy and impaired perfusion, belong to already routine ordinary procedures here. My task was to move the diagnostics of this complication to earlier stages, even before the development of the ulcers, through diagnosing an impairment in skin microcirculation. The aim of the work was to evaluate reactivity of the skin microcirculation in patients with diabetes mellitus using laser doppler flowmetry and to introduce a new method - iontophoresis. Skin microcirculation assessment has become a center of interest of many studies...
Case Study of Orthotic-Prostetics Treatment of a Patient with the Diagnosis Charcotova osteoartropatie
Kavale, Žofie ; Neuwirthová, Svatava (advisor) ; Šolc, Zdeněk (referee)
Title: Case study of ortho-prosthetic care of a patient with a diagnosis of Charcot osteoarthropathy. Objectives: The first aim of this bachelor's thesis is to summarize theoretical knowledge about the issue of diabetes and its imminent risks for the patient and the possibility of using ortho- prosthetic aids. The second aim of the work is to process a case study of ortho-prosthetic care for a patient diagnosed with Charcot's osteoarthropathy. Methods: The bachelor thesis has two parts - theoretical and practical. The theoretical part deals with the issue of diabetes mellitus and amputation of the lower limbs, as an imminent risk of this disease and prevention of amputations. The practical part consists of a case study of orthotic-prosthetic care for a patient with Charcot's osteoarthropathy who also suffers from Diabetes type 2. It describes the anamnesis, protetometry at the initial examination, production technology and a walk test with an orthosis when handing over the device to the patient. Results: The case study was completed during a six-week internship in the non-governmental medical institution Protetika s.r.o. As part of the therapy, the patient was equipped with an orthosis which after getting acquainted with the technique of walking on an orthosis, helped to increase his mobility and...
Critical limb ischemia and autologous cell therapy in diabetic foot disease, pathogenesis of Charcot osteoarthopathy.
Němcová, Andrea ; Jirkovská, Alexandra (advisor) ; Polák, Jan (referee) ; Prázný, Martin (referee)
Diabetic foot disease (DFD) is a serious complication of diabetes and, along with critical limb ischemia, significantly exacerbates the prognosis of patients. Peripheral arterial disease in patients with diabetes has an atypical clinical course, its diagnosis is challenging and is one of the most common causes of morbidity and mortality of patients with DFD. The aim of this dissertation focused on the diagnosis and treatment of DFD was to identify a suitable method for evaluating the effect of autologous cell therapy (ACT), to assess options for early diagnosis of Charcot osteoarthropathy (COA) and, possibly, to establish the association between the incidence of cardiovascular disease and DFD. In our studies concerning therapeutic vasculogenesis, we observed a significant increase in the antiangiogenic factor endostatin after ACT in contrast to its unchanged levels after standard percutaneous transluminal angioplasty; the transient increase in endostatin seems to be a marker of therapeutic vasculogenesis after ACT. A benefit of using calf muscle perfusion scintigraphy in the assessment of microcirculation and ACT effect was not clearly demonstrated. By contrast, a promising method for the evaluation of microcirculation and the effect of revascularization after ACT was MR spectroscopy of calf...
